Licensed Social Worker

• Assist in managing the continuum of inpatient medical care from admission through discharge at UAMS Health Benton/Bryant
• Complete psychosocial assessments for hospitalized patients
• Perform discharge planning, aftercare coordination, community referrals, and linkages
• Provide patient and family education, adjustment-to-illness counseling, grief counseling, and crisis intervention
• Promote patient- and family-centered care and document patient choices in the medical record
• Recommend care and services appropriate to patients’ chronological age, developmental age, social maturity, and cultural background
• Document and communicate with treatment teams, patients, and families regarding coordination of care and services
• Consult on guardianship, power of attorney, health care proxy, and advance directives
• Collaborate with UAMS departments and participate in performance improvement and regulatory compliance initiatives
• Report and review department-specific data metrics monthly; implement, monitor, and evaluate Care Management quality improvement initiatives
• Act as a resource to others, support employee development, and participate in staff recruitment and retention
• Perform other duties as assigned

• Master’s degree in social work
• Must be licensed by an Arkansas Professional Board as a Licensed Masters Social Worker or Licensed Certified Social Worker (LMSW or LCSW)
• Two (2) years of experience in an Acute Care Hospital Setting (preferred)
• Experience in Case Management (preferred)
• Certified Case Manager (CCM) or American Case Management (ACM) certification (preferred)
• Proof of legal authority to work in the United States on the first day of employment
• License or Certificate, List of three Professional References, and Resume required for application
• Subject to pre-employment screening, including criminal background, drug testing, and/or education verification

Demonstrates expertise in managing inpatient medical care, including psychosocial assessments, discharge planning, and care coordination. Proficient in patient education, crisis intervention, and compliance with regulatory standards.
