You will work under the direction of the Manager of Clinical Services and collaborate with the patient’s RN Case Manager to implement the Plan of Care. The role involves assessing patient needs, delivering therapy to alleviate anxiety, pain and distress, and supporting end-of-life living with dignity and comfort.
