Role Description
As the Nurse Practitioner, you will provide an advanced level of nursing practice to patients and families and serve as a clinical resource for staff and the interdisciplinary team.
Primary Responsibilities- Deliver specialized primary care and consultation, including prescriptive authority and on‑call duties, in collaboration with a supervising physician.
- Coordinate and manage care for patients with complex conditions, conduct comprehensive assessments, and direct interdisciplinary team meetings.
- Perform face‑to‑face patient visits and ensure proper documentation using company‑defined processes and forms.
- Provide clinical education internally and externally, serve as a resource for staff, and participate in team meetings and quality improvement initiatives.
- Support performance improvement and fulfill additional duties as assigned, contributing to overall clinical excellence.
- Current unrestricted license in the state of residence.
- Certified as a Clinical Nurse Specialist or Nurse Practitioner in the state of practice.
- Current certification by a national body such as ANCC or AANP.
- Possession of NPI number, DEA with prescribing privileges, PECOS enrollment, CAQH enrollment, Medicare number, and referable Medicaid number.
- Current driver’s license and vehicle insurance, with access to a dependable vehicle or public transportation.
- Current CPR certification.
- 1+ year of NP experience in chronic disease management, family care, or primary care.
Compensation ranges from $104,500 to $156,000. Total cash compensation includes base pay and bonus and is based on several factors including local labor markets, education, work experience, and may increase over time based on productivity and performance. We comply with all minimum wage laws as applicable.
