Become a part of our caring community. As a Center Well Home Health LPN, reporting to the Clinical Manager, you will provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes.
Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love.
Responsibilities- Provide skilled nursing care to patients onsite in their homes, under the direction of an RN.
- Implement individualized care plans in collaboration with patients, families, and the healthcare team.
- Educate patients and families on disease management, treatment options, and self‑care techniques.
- Maintain accurate records of patient care, including documentation of visits, interventions, and outcomes.
- Maintain contact with patients, physicians, clinical manager(s), and other team members promptly regarding patient or family needs and status changes.
- Participate in care coordination activities and discharge planning.
- Use your skills to make an impact.
- Current nursing license in the practicing state.
- Valid driver’s license, auto insurance and reliable transportation.
- Valid CPR certification.
- Two years experience as an LPN/LVN in a clinical setting.
- Experience in a home health or hospice setting.
This role is patient‑facing and is part of Humana’s Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB.
This role is part of Humana's driver safety program and therefore requires a valid state driver’s license. Applicants must maintain personal vehicle liability insurance with minimum limits: $25,000 bodily injury per person, $25,000 bodily injury per event, $10,000 property damage or higher per the state requirements.
Pay RatePay Per Visit/Unit: $30.58 – $42.12. Full‑time compensation range: $47,700 – $65,700 per year.
