Registered Nurse - Home Health

Job Overview

The Registered Nurse (RN) provides safe, effective, and patient-centered care while utilizing the nursing process in a short-term, insurance-approved Home Health setting. Responsibilities include evaluating the effectiveness of care delivered by other caregivers, coordinating with interdisciplinary teams, safeguarding patient rights, and maintaining confidentiality in alignment with Mercy’s mission, values, and Service Standards.

Location:

Serves patients within a 50-mile radius of Fort Smith, Arkansas, limited to the state of Arkansas.

Schedule:

Monday–Friday (8:00 AM – 4:30 PM) with occasional volunteer participation in weekend call rotation.

• Deliver professional nursing care in the home setting following physician orders.
• Develop and update individualized care plans with measurable goals.
• Monitor patient progress and communicate changes to physicians and the care team.
• Educate patients and caregivers on care plans, treatments, and medications.
• Coordinate services with interdisciplinary team members (PT, OT, ST, MSW, etc.).
• Perform treatments and procedures in compliance with nursing standards and regulations.
• Assess home safety, report concerns, including potential neglect or abuse.
• Complete accurate, timely documentation (OASIS/Medicare/Medicaid guidelines).
• Participate in on-call rotation, weekends, and holidays as needed.
• Support team collaboration, quality improvement, and patient satisfaction initiatives.

• Graduate of an accredited nursing program.
• Current RN licensure.
• Valid driver’s license.
• CPR certification.
• Preferred: 1+ year of relevant nursing experience (home health or related field).
