Home Health Registered Nurse (RN)
We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to join our team in the Fort Smith, Arkansas service area. This position is responsible for providing skilled nursing care to patients in their homes, completing comprehensive assessments, coordinating care plans, and ensuring high-quality patient outcomes. The ideal candidate will have prior home health experience and be comfortable working independently in a field-based setting.

Location:
Fort Smith, AR and surrounding coverage areas
Schedule:
Monday through Friday, 8:00 AM to 5:00 PM
Pay Rate: $51.00 - $53.00 per hour
On-Call Requirement:
One weekend per month and one weekday every other week
Mileage Reimbursement:
Reimbursed from first patient visit to last patient visit
- Provide skilled nursing services in patients' homes in accordance with physician orders and agency policies.
- Perform comprehensive patient assessments, including OASIS assessments and documentation.
- Develop, implement, and update individualized plans of care.
- Monitor patient progress and communicate changes in condition to the interdisciplinary care team.
- Educate patients and caregivers on disease management, medications, treatment plans, and safety measures.
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
- Maintain accurate, timely, and compliant clinical documentation.
- Participate in on-call rotation as scheduled.
- Ensure compliance with all federal, state, agency, and regulatory requirements.
- Active Registered Nurse (RN) license in the state of Arkansas.
- Previous Home Health nursing experience required.
- Experience completing OASIS assessments.
- Valid driver's license, reliable transportation, and proof of automobile insurance.
- Strong clinical assessment, critical thinking, and care coordination skills.
- Ability to work independently and effectively manage a field-based patient caseload.
- Current BLS/CPR certification.
- Recent experience managing a home health patient caseload.
- Excellent communication and patient education skills.
- Strong organizational and time management abilities.
- Experience with electronic medical record (EMR) systems.
- Ability to build strong relationships with patients, families, and interdisciplinary team members.
