Mercy Fort Smith is seeking a Hospital-Community RN Liaison. The role focuses on receiving Hospice, Home Health and Home Infusion referrals in acute care, physician clinics, and selected post-acute sites.
You will maintain an active presence in these settings and deliver excellent service to discharge planners, social workers, physicians, and patients. Requirements include an RN license in the applicable state, with a bachelor’s degree preferred and at least three years of nursing experience.
