Primary Care Assistant - LPN

Role:

The Primary Care Patient Care LPN collaborates with providers and the care team to deliver safe, effective, and patient-centered primary care.

Assist in driving quality outcomes, support clinical operations, and perform tasks independently within scope of practice. Duties include direct patient care, communication, and ensuring accuracy in documentation.

Graduate of an accredited practical nursing program, licensed as an LPN, with preferred healthcare experience in a primary care setting.

Teamwork, excellent communication, customer service orientation, organizational skills, flexibility, attention to detail, and self-direction.

Ability to lift 50 pounds, stand and walk for extended periods, and perform physical movements like bending, kneeling, and reaching.
