Working closely with physicians, nurses, and department leaders, you will support safe transitions, clear communication, and high-quality care across the continuum.
Required Qualifications- 2 years of Recent (within the last 3 years) Acute Care Experience. Looking for ICU, CCU, Med‑Surg or LTAC Case Management experience.
