We are seeking licensed Certified Nursing Assistants (CNA) to join our team at 611 W County Line Rd S, Fort Wayne, IN 46814.

• Shift: 6am-2pm or 2pm-10pm
• Job Type: Part Time or Full Time
• $1000 Sign‑on Bonus for Full‑time employees – $500 Sign‑on Bonus for Part‑time employees
• $2.00 Shift Differential for 2nd or 3rd Shift

• Lead and provide 1440 care.
• Review Resident Care Plans at start of shift.
• Bathe and clean both male and female residents as needed.
• Assist in oral, dental, and grooming tasks for residents.
• Assist residents in dressing and encourage appropriate clothing.
• Make beds, change linens, and keep rooms tidy.
• Feed residents and assist with meals and snacks.
• Answer call lights promptly.
• Assist with toileting, changing incontinence products, and cleaning.
• Take vital signs and report abnormalities to the charge nurse.
• Measure intake and output, and assist with specimen collection.
• Ensure water is accessible and clean pitchers per schedule.
• Clean wheelchairs, walkers, commodes, and other equipment.
• Report unusual excretion colors or odors.
• Assist in transfer, ambulation, and ROM exercises.
• Maintain resident dignity and privacy, following the Resident Bill of Rights.
• Apply and monitor safety devices.
• Check residents unable to call for help regularly.
• Prepare rooms for new admissions.
• Assist with admission, transfer, and discharge processes.
• Escort newly admitted residents to their rooms.
• Complete Nurse Aide flow sheet during shift.
• Document care in Care Tracker.
• Follow universal precautions and isolation procedures.
• Inventory personal possessions as instructed.
• Report occupational exposures immediately.
• Maintain clean service areas and clear halls.
• Report accidents or incidents immediately.
• Report suspected abuse immediately.
• Perform other duties as assigned.

• Professional in attitude, demeanor, and dress.
• Good organizational skills and ability to work under pressure.
• Subject to frequent interruptions.
• Involved with residents and associates under all conditions.
• Subject to potentially upset residents, family members, associates, and visitors.
• Varied working hours to include days, evenings, weekends, and holidays as needed.
• Subject to callback during emergency conditions.
• Continuous physical movement.
