Position Overview

The Social Services Director will be traveling to various Majestic Care locations in the Indianapolis region. This person may or may not be visiting multiple buildings in the same week and could be spending up to 45 days in the same location at a time. They will direct and manage the day-to-day operations of the Social Service department in accordance with current federal and state regulations in addition to the community policy.

Key Responsibilities
  • Supervise the social services department in the day-to-day operations ensuring to set job assignments and set priorities.
  • Develop goals and objectives for the department; motivate and manage care team members to accomplish the same.
  • Ensure new care team members are effectively oriented; ensure an ongoing educational program is in place.
  • Develop positive relationships on behalf of the community with regulatory agencies, residents, and families including participating during family orientation and family council.
  • Assist in preparing budget for the department; administer approved budget.
  • Implement policies/procedures with follow-up and supervision of care team members to ensure compliance.
  • Perform resident assessments at admission, quarterly, upon condition change, and/or annually.
  • Create, review, and update care plan and progress notes.
  • Provide direct psychosocial intervention.
  • Coordinate resident visits with outside services, dental, optical, etc.
  • Attend and document resident council meetings.
  • Assist resident families in coping with skilled nursing placement, physical illness, and disabilities of the resident as well as the grieving process.
  • Work with the resident, family, and other care team members to plan discharge.
  • Conduct in-service programs to educate staff regarding psychosocial issues and patient rights.
Qualifications
  • Minimum of a Bachelor’s degree in social work; OR
  • Minimum of a Bachelor’s degree in a human-services related major, including sociology, gerontology, psychology, rehabilitation counseling or special education; AND
  • One year in a healthcare setting where the individual is working as a supervised social worker who works directly with individuals
