Physical Therapist (PT) / Occupational Therapist (OT)

We are seeking dedicated and compassionate Physical Therapists and Occupational Therapists to join our team at Community Home Health Care & CIHC in Fort Wayne, IN. In this role, you will provide exceptional rehabilitative care to patients in their homes, helping them improve their mobility, independence, and overall quality of life.

• Conduct thorough patient evaluations to develop individualized treatment plans.
• Deliver personalized therapy sessions focusing on improving strength, coordination, and daily living skills.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Monitor patient progress and adjust treatment plans as necessary.
• Educate patients and their families on therapeutic techniques and health maintenance.
• Maintain accurate and timely documentation in compliance with agency and regulatory standards.

• Licensed Physical Therapist or Occupational Therapist in the state of Indiana.
• Strong clinical skills with a passion for home health care.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage time effectively in a home care setting.
• Valid driver’s license and reliable transportation.
