Physical Therapist – Local Skilled Nursing Facility

We are seeking a Physical Therapist to work collaboratively with the therapy teams and the Rehab Department within a Skilled Nursing Facility.

• Complete evaluations and all necessary documentation.
• Provide recommendations supporting communications/assessments.
• Deliver therapy as outlined in treatment plans and prescriptions.
• Offer treatments that help improve patient inabilities and/or challenges.
• Independently manage workflow, including allocating time for treatment documentation of individual and group treatments and notes in the electronic records system.
• Familiar with PDPM and align therapy treatments and plans with an understanding of Medicaid and Medicare billing.

• New Jersey license.
• Background/experience in long‑term care, specifically a Skilled Nursing Facility.
• Strong organizational, interpersonal, and communication skills.
• Ability to manage patients with different personality types and ability levels.
