ResponsibilitiesConduct comprehensive patient evaluations, develop and modify individualized treatment plans, and collaborate with interdisciplinary teams to facilitate patient recovery and discharge planning. Manage individual and group therapy sessions, document medical records accurately, and participate in patient education, safety protocols, and organizational improvement initiatives.
Qualifications & SkillsValid state license, BLS certification, minimum of 2 years of experience, and LTACH experience preferred. Ability to perform clinical reasoning, work efficiently, and adhere to regulatory and safety standards. Commitment to ongoing professional development and supervision of students where appropriate.
Key DutiesEvaluate patients, develop treatment strategies, address discharge barriers, manage treatment sessions, document records, and collaborate with healthcare teams to ensure high-quality patient care.
Additional InformationFlexible scheduling Monday to Friday, 8:00 AM to 5:00 PM, with potential weekend rotations.
