We are seeking Registered Nurses (RN) to provide skilled nursing visits to clients in their homes. This role offers flexibility, independence, and the opportunity to deliver focused, one-on-one care.

Position Type

• PRN or Part-time
• Home health visits

Responsibilities

• Perform skilled nursing visits per physician orders
• Complete patient assessments and ongoing evaluations
• Provide treatments, medication management, and patient education
• Document visits accurately and timely in the clinical system
• Communicate changes in patient condition to the clinical team
• Maintain compliance with state and agency standards

Requirements

• Active RN license in good standing
• Home health experience preferred (training available)
• Reliable transportation and valid driver’s license
• Strong clinical judgment and documentation skills
• Professional, dependable, and patient-focused
