Summary:
Responsible for ongoing evaluation of activities, services, and rehabilitation pathways beginning at the point of anticipated admission to the Rehabilitation Center through discharge. Responsible for coordination and documentation of conferences, patient and family teaching, and insurance or other third party payer requirements. Involved in the orientation of patient, significant other, and/or family to the rehab program. Responsible for ongoing communication and utilization review to demonstrate necessity of care.
- Provide ongoing evaluation of activities, services, and rehabilitation pathways from anticipated admission through discharge.
- Coordinate and document conferences, patient and family teaching, and requirements of insurance or other third party payers.
- Orient patients, significant others, and/or family to the rehabilitation program.
- Maintain ongoing communication and utilization review to demonstrate the necessity of care.
Must be a graduate of professional school of nursing and hold at a minimum an Associate of Science in Nursing (ASN). Must obtain Bachelor of Science in Nursing (BSN) within three years of employment or will not be eligible to continue in the role. Coursework is completed outside of work hours.
Licensure/CertificationMust provide current RN license by Indiana State Board of Nursing. CRRN certification preferred.

ExperienceExperience in acute rehabilitation program preferred.
Other QualificationsMust be able to lift up to 50 pounds.
