LPN/RN

We are seeking a dedicated Overnight Nurse to join our team at Community Home Health Care, providing compassionate and high-quality care to patients during the overnight hours. The ideal candidate will work the shift from 10pm to 6am in the southwest area of Fort Wayne, IN, ensuring the comfort and safety of our clients throughout the night.

Responsibilities
  • Provide skilled nursing care to patients in their homes during the overnight shift.
  • Monitor patient health and communicate any changes or concerns to the appropriate healthcare team members.
  • Administer medications and treatments as prescribed.
  • Maintain accurate and timely documentation of patient care and observations.
  • Assist with patient hygiene, mobility, and comfort as needed.
  • Respond promptly to patient emergencies and provide appropriate interventions.
Requirements
  • Current Registered Nurse (RN/LPN) license in the state of Indiana.
  • Experience in home health nursing or overnight care preferred.
  • Strong assessment and critical thinking skills.
  • Excellent communication and interpersonal abilities.
  • Reliable transportation to travel to patient homes in the southwest Fort Wayne area.
  • Ability to work independently during overnight hours (10pm-6am).

Registered Nurse (LPN/RN)

We are seeking an experienced Registered Nurse (LPN/RN) to support and oversee high-quality in-home patient care. This role is responsible for clinical oversight, quality assurance, patient and caregiver satisfaction, and effective coordination of care across multiple locations.

Key Responsibilities
  • Utilize effective problem-solving and clinical decision-making skills to identify issues, determine root causes, develop solutions, and evaluate outcomes
  • Conduct client visits and participate in quality improvement initiatives to ensure service satisfaction
  • Investigate and resolve assigned patient and staff complaints in a timely and professional manner
  • Ensure all clinical documentation is accurate, complete, timely, and compliant with company policies, procedures, and regulatory requirements
  • Assess patient status and interpret clinical data to identify age-specific and diagnosis-related care needs
  • Coordinate and communicate effectively with patients, families, caregivers, and interdisciplinary care teams
  • Provide oversight and guidance to clinical staff as needed to support quality patient outcomes

Qualifications
  • Active and unrestricted Registered Nurse (LPN/RN) license in the state of Indiana
  • Valid driver’s license and reliable personal vehicle

  • Ability to assess patient data and identify appropriate care needs based on age and diagnosis
  • Strong communication, organization, and problem‑solving skills
  • Proficient computer skills and experience with electronic documentation systems
