Experience:

Certifications:
BLS (American Heart Association), Current automobile insurance and valid driver's license
Must-Have:
Knowledge of terminally ill patients and their families, excellent patient assessment skills, and ability to apply knowledge of hospice care needs
Description:
The Registered Nurse (RN) will provide direct patient care services to hospice patients.
Supportive care will be provided to the patient's family.
The RN will manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff.
The goal is to enhance the quality of hospice care and case manage all assigned patient clinical records.
The RN assures the highest quality of care is provided and documented in accordance with the Plan of Care.
Initial and ongoing assessments of the patient will be performed.
The RN will help develop and implement the Hospice Plan of Care as prescribed by the physician.
Collaboration with other members of the interdisciplinary team will ensure all-inclusive care, including physical, psychosocial, educational, emotional, and spiritual services.
Bereavement support will also be provided to the patient and family.
Any changes in the patient’s condition must be reported to the Attending Physician and Hospice Physician.
The RN will maintain an accurate and updated medical record, including all interdisciplinary notes and relevant care details.
Infection control measures and standard precautions will be practiced.
The RN will monitor the scheduling of all staff and may participate in the agency on-call schedule.
Participation in the Quality Assessment and Performance Improvement Program is expected.
The RN will promote the company’s core values consistently.
Attending staff meetings is required.
A high level of knowledge pertaining to pain and symptom management in the dying process must be maintained.
Routine supervisory visits with all other team members will occur at least monthly.
Ongoing reviews and follow-ups of assigned patient clinical records will assure quality services are delivered.
Maintaining close contact with the patient’s family or significant others will provide necessary information and support.
The RN will be the primary liaison between the physician, patient/family, and hospice team.
Education for the patient, family, or caregivers will be provided concerning the needs identified in the Plan of Care.
This includes medication administration, dressing changes, oxygen administration, hospice philosophy, and care of the terminally ill patient.
