Compensation is based on total years of experience in previous clinical roles.

In this role, you'll care for patients in their homes in the Allen, Wells, and Huntington County area.

Both RN and LPN are encouraged to apply. 32 hours per week.

You will be working 32 hours each week, Monday through Friday, with Weekend rotations.

REPORTS TO:
Clinical Team Leader

POSITION OBJECTIVE: Performs nursing procedures and care to assigned terminally ill patients by assessing needs and implementing nursing care plans under the coordination of the RN Case Manager.

• Graduate of an accredited school of nursing.
• Bachelor’s degree in Nursing preferred.
• Must have a current RN license to practice in the state of Indiana.
• Must hold current CPR certification or obtain within the orientation period.
• Must hold current TB certification or obtain within the orientation period.
• Must be able to provide those services requiring substantial specialized nursing skill.
• Prefer one (1) year of recent hospital or home health care experience.
• Must have a current motor vehicle operator’s license and have access to an automobile with current automobile insurance coverage.
