As an important member of our interdisciplinary team, the LPN delivers quality nursing care to patients through interpersonal contact and provides care and services to help patients attain or maintain the highest practicable physical, mental, and psychosocial well-being in accordance with applicable laws, regulations, and Life Care standards. You will monitor and report changes in patient conditions and, on a daily basis, assign duties, instruct, and provide clinical supervision to CNAs.

• Deliver quality nursing care to patients as part of an interdisciplinary team.
• Monitor patients’ conditions, report changes, and document findings accurately.
• Assign duties, provide instruction, and supervise CNAs on a daily basis.

• Graduated from an accredited LPN program.
• Currently licensed/registered in the applicable state. Must maintain an active LPN/LVN license in good standing throughout employment.
• One (1) year skilled nursing experience preferred.
• CPR certification upon hire or during orientation; CPR certification must remain current during employment.

• Make independent decisions when circumstances warrant such action.
• Deliver knowledgeable and competent quality nursing care to patients.
• Exhibit excellent customer service and a positive attitude towards patients.
