Nurse Practitioner - Primary Care

Below are the primary duties and essential functions of an employee in this position. While not all tasks may be performed by every employee in this role, all employees are expected to perform related tasks as needed. • Provides patient assessment,diagnosis and treatment plansin accordance with statutes,regulations and protocols regulating the profession.
• Collaborates with the Medical Doctor, staff psychiatrist, nursing staff, therapists and other members of the clinical team.
• Provides primary care health services, education,counseling, and emotional support.
• Issues medication prescription sin accordance with treatment guidelines.
• Orders laboratory tests,interprets,and explains the test results to patients.
• Provides patient education regarding medications, risks,benefit sand reasonable outcome expectations.
• Communicates with patients and engages infollow-up as necessary.
• Other duties as assigned.

Physical Requirements:

• Occupational Exposure Risk Classification:
  The normal work routine involves moderate exposure to blood, body fluids,or tissues.
• The normal work routine has aminimal ergonomic risk for repetitive motion impairment.
• The position may require aminimal amount of physical exertion (bending, lifting, twisting) in the work area.

Required Qualifications:

• Education:
  
  Master’s degree in nursing.
• Certificate or Professional License:
• Current Certification or eligible to be certified by the American Nurses Credentialing Center.
• Certified as a Family Nurse Practitioner (FNP).
• Currently holding or eligible to obtain Prescriptive Authority in Indiana.
• CPI (Crisis Prevention Intervention)
  -certification must be obtained afterhire and prior to providing direct patient care. This certification must bemaintainedand kept current through employment.
• BLS (Basic Life Support) and First Aid
  -certification must be obtained afterhire and prior to providing direct patient care.
  
  This certification must bemaintainedand kept current through employment.
• Valid driver’s license and active auto insurance coverage.
• Satisfactory results from criminal and driving record background checks
