Licensed Practical Nurse

Kingston Healthcare is seeking a Licensed Practical Nurse to join our Healthcare & Medical Services team in Vermilion, OH. At Kingston Health Center of Vermilion, you'll provide high-quality nursing care for residents in assisted living, rehabilitation, and memory care settings. You will administer medications, monitor patient conditions, support care plans, and collaborate with an interdisciplinary team. Responsibilities

• Provide direct, resident-centered nursing care in assisted living, rehabilitation, and memory care settings
• Administer medications and treatments in accordance with physician orders and facility policies
• Monitor and document vital signs, assessments, and changes in resident condition, reporting concerns promptly
• Assist with wound care, injections, catheter care, and other procedures within LPN scope of practice
• Participate in developing, implementing, and reviewing individualized care plans
• Educate and support residents and families regarding care plans, medications, and health goals
• Promote resident safety, infection control, and adherence to regulatory and facility standards
• Collaborate with RNs, physicians, therapists, social workers, and other team members to coordinate care
• Supervise and provide direction to nursing assistants to ensure high-quality daily care
• Maintain accurate, timely documentation in electronic health records and contribute to quality improvement efforts
