Job Description- Hospice QA Nurse
- Schedule:
Monday-Friday 8:00am - 5:00 pm - Demonstrate commitment to the Paradigm Health mission, vision and core values
- Reviews hospice documentation for accuracy, compliance, and eligibility in a timely manner.
- Communicates compliance concerns and/or trends with the Compliance Coordinator.
- Participates in continuing education to maximize awareness of changing regulations and industry standards.
- Reviews hospice admissions/recertifications to ensure orders, diagnosis, and eligibility are thorough and accurate from intake through the admission process.
- Reviews all hospice admissions to ensure the comprehensive assessment is documented and completed in the appropriate time frames.
- Reviews all hospice orders to ensure compliance with CMS, AO, and State regulations.
- Works closely with medical records to determine if all signatures have been obtained as required by regulations for all orders, plans of care, etc.
- Reviews all hospice documentation including but not limited to: admission, orders, F2
Fs, written certifications/recertifications, recertifications, HOPE documentation, visit notes, communication notes, discharges, discharge summaries, for all disciplines, to ensure compliance with regulations. - Holds the Hospice staff accountable by auditing and monitoring their documentation.
- Completes documentation review in a timely manner and communicates follow-up needs with staff in a professional, constructive, and educational tone to ensure compliance.
- Monitors documentation practices and proactively provides guidance and support to hospice staff to improve accuracy, completeness, and compliance.
- Collaborates with Hospice Clinical Coordinators to identify educational needs and facilitate targeted staff education and ongoing performance improvement.
- Works closely with the biller to ensure all documentation in reviewed and completed in a timely manner for billing purposes.
- Assists the Hospice Clinical Coordinator in the new hire orientation process for new Hospice nurses by providing reading material and documentation training.
- Adheres to all organization, payroll, billing and documentation policies and procedures.
- Supports Clinical Coordinator and hospice team during IDG meetings.
- Ensures timely generation of plan of care orders and reviews for continuity between comprehensive assessment and plan of care.
- Consults on difficult situations to ensure the Paradigm Procedures are utilized while maintaining compliance with all state, federal and AO standards.

Required:
- Minimum 2 years of hospice experience, ideally in quality, compliance, or clinical documentation roles.
- Excellent communication, training, and interpersonal skills with the ability to communicate effectively with hospice staff in a positive and professional manner.
- Strong attention to detail with a high degree of accuracy in documentation and data review.
- Ability and willingness to travel to hospice sites as needed and on a quarterly basis.
- Valid state driver’s license and reliable automobile.
