Summary
The Licensed Practical Nurse has the responsibility and accountability for assuring delivery of quality nursing care for hospice and home care patients under the direction of a Registered Nurse in accordance with individual patient needs, physician orders, hospice and departmental policies. The LPN will provide hospice/palliative and home care services with respect for human dignity and the uniqueness of the patient, unrestricted by consideration of socio-economic status, personal attributes or the nature of the health problems.
Maintains confidentiality of information regarding patient/caregiver, and all health care team members.
Must be a graduate of a school of nursing. Must complete hospital general orientation and department orientation (Hospice).
Licensure/CertificationMust possess a current and valid license as a Licensed Practical Nurse from the Indiana State Board of Nursing. Must have valid state driver's license, reliable transportation, and proof of auto insurance. Must have current CPR certification.
ExperienceMust have two years of experience in home care, oncology, critical, med/surg or nursing home.
