Bright Star Care of Fort Wayne is seeking a compassionate Licensed Practical Nurse (LPN) to provide one-on-one private duty skilled nursing care in clients’ homes in Fort Wayne, IN.
This opportunity allows you to focus on an individual patient, learn their routines, and develop a trusting relationship with their family. You’ll work independently in the home with support from experienced clinical leadership.
Pay and Scheduling- $30.00-$37.00 per hour, based on experience, credentials, and shift requirements
- Full-time, part-time, and PRN assignments in Fort Wayne, IN
- Day, evening, night, and weekend opportunities

- Current, unrestricted Indiana Licensed Practical Nurse (LPN) license
- Current CPR/BLS certification
- Ability to complete required background and employment screenings
- Ability to meet agency health, competency, and credentialing requirements
- Reliable transportation to the client’s home
- Strong clinical observation, communication, and documentation skills
- Ability to work independently while staying connected with the clinical team
- Dependability, sound judgment, and respect for patient privacy
- Private duty nursing or home health care
- Caring for medically complex patients
- Tracheostomy care, suctioning, oxygen therapy, or enteral feeding
- Managing complex medication regimens
- Respiratory, pediatric, step-down, or long-term acute-care experience
- Additional specialty training relevant to assigned clients
As a Licensed Practical Nurse (LPN), you will provide care according to physician orders and the individualized plan of care, within your scope of practice and demonstrated competencies.
- Deliver personalized, one-on-one skilled nursing care.
- Monitor vital signs, symptoms, and patient responses.
- Complete nursing observations and assessments within your scope.
- Administer prescribed medications and treatments.
- Provide ordered respiratory care, including tracheostomy care, suctioning, and oxygen therapy.
- Administer enteral feedings and medications as ordered.
- Perform wound care and other assigned nursing interventions.
- Recognize changes in condition and promptly notify the appropriate clinician.
- Follow emergency procedures when urgent concerns arise.
- Complete accurate, timely clinical documentation.
- Communicate with families, physicians, therapists, case managers, and agency leadership.
- Reinforce patient and caregiver education.
- Contribute observations to care-plan updates.
- Follow infection-control procedures, safety standards, and HIPAA requirements.
