As a Clinic Nurse (RN), you will:
- Coordinate and manage patient care plans across healthcare providers and services.
- Assess patients’ clinical, psychosocial, and functional needs to identify appropriate interventions.
- Collaborate with physicians, nurses, social workers, insurance companies, and community resources.
- Monitor patient progress, outcomes, and barriers to care while adjusting plans as needed.
- Advocate for patients by promoting quality care, safe transitions, appropriate resource utilization, and continuity of care.
A few must-haves for Clinic Nurses:
- Graduate of an accredited nursing program with an active RN license in Indiana
- Minimum of 2+ years of experience as an RN Case Manager
- Current CPR certification from the American Heart Association
- Prefer certification in Case Management
- Strong assessment skills, critical thinking, and the ability to work independently
- Compassionate communicator who values collaboration with students, staff, and families
