FLSA and Employee Status: Non-exempt, Full time
Pay Range : 18.00 - $22.00 Hour Position Summary: At Catholic Charities of the Diocese of Fort Wayne-South Bend, we have been serving the most vulnerable populations in our communities for over 100 years. Within our 20+ offered programs, we have specific programs that serve the community at large on a case-by-case basis. As a Community Services Case Manager, you will have the opportunity to impact the lives of community members in need of rent and utility assistance, pregnancy support, human trafficking victims assistance, and/or homeless services support.

You will serve clients in need by offering them a variety of resources to improve their unique situations.
Essential Duties/Responsibilities:

  • Provide case management services and related support to adults and children in various programs within the agency that may include but are not limited to:
    • Resource & Referral Services
    • Education Creates Hope and Opportunity (ECHO)
    • Pregnancy and Parenting Services
    • Human Trafficking Services
    • Homeless Shelter Services
    • Thrive Program
  • Communicate effectively with clients and have a solid understanding of how to work with people from diverse backgrounds.
  • Provide direct case management services to clients in office, home, community or mobile unit settings including assessments, case planning, advocacy, referral, crisis management, skills development and problem solving as needed.
  • Assist pregnant and parenting teens to address barriers in completing their high school education or high school equivalency program and/or post-secondary program completion.
  • Provide direct services to clients in need of emergency financial or material assistance or enrolled in self-sufficiency programming.
  • Facilitate one on one or small group skills building or training for clients including budget review, financial literacy, banking, job skills development, job search and interviewing practice, school enrollment, FAFSA completion, etc.
  • Provide case management services to residents transitioning out of the Rescue Mission.
  • Provide direct service and coordinate activities for clients impacted by disasters.
  • Develop and maintain relationships with community service providers in order to coordinate client services.
  • Promote and represent the programs in the community and with clients.
  • Attend relevant community service trainings, committee and service provider meetings.
  • Ensure that all case documentation and case records are current and accurate.
  • Run reports, collect data, and prepare grant or program reports as needed.
  • Comply with all grant, funder and audit requirements of the program.
  • Perform related functions necessary to support the mission and core values of Catholic Charities.
Qualifications:
  • Bachelor’s degree in social service or related field from an accredited college/university and a minimum of one year work experience in social service setting.
  • Working knowledge of community service and public program providers.
  • Working knowledge of CORE 40/ HSE requirements, college application & entrance requirements and FAFSA application process.
  • Working knowledge of employment skills development and financial literacy.
  • Ability to work independently and effectively to provide services with limited supervision.
  • Ability to effectively represent the program in public situations.
  • Excellent written and oral communication skills.
  • Ability to travel in personal vehicle within the East Region on a regular basis and within the Diocese of Fort Wayne South Bend, Inc. as needed.
Conditions Of Employment:
  • Have a valid driver’s license with a clean driving record.
