Case Manager for McKinney-Vento Eligible Families
Eight hour fifty two week full time temporary position with benefits; position dates: 08.01.26 through 05.25.2027. You will work with McKinney-Vento families.

Basic Function
Fort Wayne Community Schools is partnering with United Way of Allen County to put in place a case manager to provide wrap-around and specialized service and continuity of care during their time as a McKinney-Vento eligible family. Under the direction of The Families in Transition Coordinator, the Families in Transition Case Manager will serve as a liaison between teachers, parents, students, support staff, administrators, and the community regarding educational programs, services, and the issues and problems of identified homeless students in areas such as attendance, intervention, behavioral modification, emotional growth, academics, and health;
provide related outreach, support, and guidance services. Collaborate and coordinate resources with students and families experiencing homelessness to ensure access to services such as therapy, food bank resources, and medical appointments in a timely manner.

Qualifications
Bachelor's degree in social work or human services, preferred area of studies. Employee must have strong planning, organizational, analytical, computer, accountability, and oral and written communication skills. Experience working with students or families in an educational, social service or related environment. The employee must have experience in providing supports to vulnerable populations. Incumbents must possess knowledge of the comprehensive organization, operations, policies and objectives of assigned department or area.
Incumbents must be able to transport self to locations away from his or her office as required. Must have Indiana Driver's License.

Essential Functions
• Conduct a Homeless Education Needs Assessment within the consortium to determine the needs of homeless children and youth, as well as to assess LEA capacity to comply with the MV Law and grant requirements, and to map community resources.
• Build support and partnership across the consortium region among human service agencies, faith-based organizations, related service providers, schools, and businesses to facilitate awareness and understanding of homeless issues, as well as coordination of services and supports to homeless students.
• Assist with enrollment to ensure homeless students have a full and equal opportunity to succeed in school.
• Maintain weekly phone contact, biweekly face to face sessions, and home visits when necessary with each family assigned to caseload.
• Collaborate with professionals and families to execute their care plan as directed by the Families in Transition Coordinator
• Listening to clients concerns and providing validation as required.
• Following up with discharge clients provided as well as incidents competent in communication with diverse populations.
• Attend/Assist monthly Families in Transition meetings.
• Ensure appropriate record keeping while maintaining confidentiality of all recipient information.
• Responsible for completing and maintaining all necessary documentation for services. Collect and record data on progress/trends and provides data to Families in Transition Coordinator weekly.
• Collaborate with other consortium school district personnel to ensure children and youth experiencing homelessness are identified through coordination with outreach to other community agencies.
• Cultivate resource lists and provide referrals for homeless families and children to heath care services, dental services, mental health services, substance abuse services, housing services and other appropriate services located in the community and state.
• General understanding of diversity and in-person environment.
