Somewhere in Fort Wayne tonight, a parent is putting their kids to sleep in a place that is not a home. We are hiring someone who wants to help change that.

Vincent Village provides shelter, care, advocacy, affordable housing, and support services for families experiencing homelessness. For forty years we have helped families build strength and reach lasting independence. We are looking for a Family Transition Specialist with the judgment, persistence, and relationship-building skills to help families move from crisis toward a front door of their own.

You have done hard work with people before. The best person for this job may not have the words “case manager” anywhere on their resume.

Maybe you have carried a caseload and know how a benefits application, a landlord, and a school enrollment fit together. Maybe you spent years in a classroom or a counselor's office with families who were barely holding on. Maybe you coached, ran a youth program, or led a ministry, and you know how to get people to do hard things because they trust you.

Maybe you have experienced housing instability yourself and have wanted ever since to be the person on the other side of that desk. That experience matters here, too.

All of it counts. What we need is someone who has spent real time doing hard work with people and can show us what they learned, what they accomplished, and how they helped people move forward.

And we pay for those years. This position starts at $50,000. If you have a record of helping families navigate hard circumstances and move toward stability, we want to hear about it.

This work is meaningful, but it is not easy. Families may arrive frustrated, wary, overwhelmed, or exhausted. Progress is rarely a straight line. You will celebrate breakthroughs, navigate setbacks, hold boundaries, solve problems that don't have obvious answers, and keep showing up. If that kind of work gives you a sense of purpose rather than making you back away, you may be the person we're looking for.

• Earn a family's trust, and keep it when things get complicated.
• Bring a trauma-informed approach to your work, recognizing that guardedness, distress, or an outburst may be rooted in what a family has experienced. Meet those moments with curiosity, consistency, appropriate boundaries, and a focus on understanding rather than judgment.
• Set meaningful goals with families, monitor progress, address barriers, and provide consistent encouragement and accountability.
• Find the second and third solution when the first one falls through.
• Manage a full caseload, document it well, prioritize competing needs, and take initiative while keeping supervisors and team members informed.

• Meet one-on-one with families to assess needs, build individualized transition plans, and track progress toward permanent housing.
• Help families build the skills and confidence they need for lasting independence, and walk alongside them through the appointments and moments that matter.
• Open doors to housing, health care, child care, and employment opportunities-and advocate persistently when those doors don’t open easily.
• Keep every family's story documented in the Homeless Management Information System (HMIS) and in our other data systems with accuracy and discipline. The information you collect and enter helps us understand what works, demonstrate our impact, and keep the program funded.
