Family Medicine - Geriatrics

Acclaim Physician Group, Inc. Outpatient clinic – Patient panel – Primary Care – Geriatrics.

• Coordinate multidisciplinary care delivered to panel of patients to optimize patient care.
• Review patient medical records to diagnose, treat, and provide care management and education to patients and families.
• Make appropriate patient referrals.
• Champion a culture of empathy, dignity, and respect for aging populations.
• Monitor national health guidelines and those established by JPS Health Network and Acclaim Multispecialty Group for the management of patient care.
• Provide evidence‑based and compassionate care to all patients as assigned to improve patient outcomes and increase patient satisfaction scores.
• Maintain a collaborative environment with other referring physicians, residents, medical students, advanced practice professionals, staff, and support staff to provide quality patient care.
• Support academic, educational and research endeavors.
• Assess and manage chronic and acute conditions in patients with multiple comorbidities.
• Partner and communicate with acumen to build a positive brand and patient/customer experience for those who encounter JPS Health Network and Acclaim Multispecialty Group.
• Serve as a resource to the community.
• Perform other job‑related duties as assigned.

• Current Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree from an accredited school of medicine.
• Residency and/or Fellowship training in Family Medicine (Geriatrics).
• Current, unrestricted medical license to practice in the State of Texas.
• Board Certification or Board Eligible.
• Unrestricted work authorization to work in the United States.

• Bilingual or multilingual proficiency is a plus.
• Prior experience in post‑acute or long‑term care settings strongly preferred.
• Proficient with electronic medical records and knowledgeable of CMS/SNF regulatory requirements.

• Comfortable working in a large public/county facility.
• Working knowledge in assigned area, and competency in assessment techniques, diagnosis and treatment processes that apply to the assigned medical specialty area.
• Good interpersonal, verbal and oral communication skills to work effectively with other health care professionals and serve as a resource to the community.
• Self‑directed and take initiative to develop and participate in community activities and medical outreach programs pertaining to the medical specialty area.
