Description:
The Pharmacist - Inpatient is responsible for providing quality care to patients through both clinical and operational pharmacy services in accordance with standards of professional practice, state and federal laws, and standards established by regulatory or accreditation agencies. Typical Duties:
* Interprets, clarifies and transcribes or verifies all medication orders accurately and timely into the pharmacy computer system and electronic medical record.
* Reviews patient profiles to ensure appropriate drug selection and dosage, and monitors drug therapy regimens for contraindications, allergies, drug-drug and drug - food interactions, critical lab values and therapeutic duplications.
* Ensures all medications are prepared and labeled accurately and efficiently, prioritizing work flow to ensure delivery complies with the facility's medication turn-around time policy.
* Supports the hospital formulary by utilizing the hospital's therapeutic substitution protocols, promoting rational drug therapy selection, and minimizing procurement of non-formulary medications.
* Provides accurate and timely drug information and education to patients, families, nursing and the medical staff.
* Documents medication order clarifications and clinical interventions and recommendations accurately and completely per established protocol.
* Detects and reports suspected adverse drug events accurately and in a timely manner.
* Effectively supervises pharmacy operations activities, including pharmacy technicians, and provides support to the entire pharmacy team. Participates in the continuous quality improvement of pharmacy operations.
* Ensures compliance with all regulatory rules and regulations, including USP and controlled substance standards.
* Provides basic technical support for automated dispensing machines and other pharmacy computer systems and printers.
* Performs other related job duties as assigned.
Qualifications:
Required
Education and Experience:
* Bachelor of Pharmacy (BS Pharm) or Doctor of Pharmacy (Pharm.
D.) from an accredited College of Pharmacy. Preferred
Education and Experience:
* 2 plus years of direct patient care that demonstrates medication management. Required
Licensure/Certification :
* Must have current licensure from the Texas State Board of Pharmacy or proof of reciprocity of licensure between the State of Texas and another state.
