About the RoleThe Licensed Social Worker plays a critical role in providing comprehensive support and advocacy for individuals and families facing a variety of social, emotional, and economic challenges. This position involves assessing client needs, developing personalized care plans, and connecting clients with appropriate community resources and services to promote their well‑being and self‑sufficiency. The social worker will collaborate closely with healthcare providers, educators, and legal professionals to ensure a holistic approach to client care.
Additionally, the role requires maintaining detailed documentation and case notes to track progress and outcomes effectively. Ultimately, the Licensed Social Worker aims to empower clients to overcome barriers and improve their quality of life through compassionate and informed intervention.
- Valid and current license to practice social work (LCSW, LMSW, or equivalent) in Texas.
- Bachelor’s or Master’s degree in Social Work from an accredited institution.
- Demonstrated experience in case management, client assessment, and counseling.
- Strong knowledge of social service systems, community resources, and relevant laws and regulations.
- Excellent communication, interpersonal, and organizational skills.
- Minimum 1 year experience in LTC/SNF.
- Master’s degree in Social Work (MSW) with clinical licensure (LCSW).
- Experience working with diverse populations including children, elderly, and individuals with disabilities.
- Training or certification in trauma‑informed care, crisis intervention, or substance abuse counseling.
- Familiarity with electronic health records (EHR) and case management software.
- Bilingual abilities or proficiency in a second language relevant to the community served.
- Conduct thorough assessments of clients’ social, emotional, and environmental needs to develop individualized care plans.
- Provide counseling and support to clients and their families to address mental health, substance abuse, and other psychosocial issues.
- Advocate on behalf of clients to secure access to healthcare, housing, employment, and social services.
- Collaborate with multidisciplinary teams including healthcare providers, educators, and legal professionals to coordinate comprehensive care.
- Maintain accurate and confidential case records, prepare reports, and comply with all regulatory and ethical standards.
- Facilitate crisis intervention and provide referrals to specialized services as needed.
- Participate in community outreach and education programs to raise awareness about available social services.
The Licensed Social Worker utilizes strong assessment and analytical skills daily to identify client needs and develop effective intervention plans. Communication skills are essential for building trust with clients and collaborating with multidisciplinary teams to coordinate care. Organizational skills enable the social worker to manage multiple cases simultaneously while maintaining detailed and accurate documentation. Problem‑solving abilities are applied to navigate complex social systems and advocate for client access to resources.
Additionally, cultural competency and empathy are critical in delivering respectful and individualized support to diverse client populations.
