What You’ll Do:

As a DBMD Case Manager
, you’ll play a vital role in coordinating and monitoring services for individuals with Deaf Blind Multiple Disabilities. You’ll ensure that each client receives the care and support they need to thrive.

Key Responsibilities:

  • Oversee day-to-day activities and service delivery for your assigned caseload.
  • Ensure compliance with DBMD program guidelines and licensure standards.
  • Maintain accurate and timely documentation and billing.
  • Build strong, supportive relationships with clients, families, vendors, and team members.
  • Participate in training, quality improvement, and on-call duties as needed.
  • Travel locally for client visits and community engagement.

What You Bring:

We’re looking for someone who is not only qualified but also deeply committed to making a difference.

Qualifications:

  • Education and experience in health and human services (see full criteria below).
  • Proficiency in communication methods used by individuals with disabilities (e.g., ASL, tactile symbols).
  • Strong computer skills (MS Word, Excel, database software).
  • Reliable transportation, valid TX driver’s license, and auto insurance.
  • CPR and First Aid certification (in-person only).
  • Ability to type 35 WPM with 80% accuracy and demonstrate Excel proficiency.

Education/Experience Options:

  • Bachelor’s degree + 2 years of direct service experience, OR
  • Associate’s degree + 4 years of experience, OR
  • High school diploma + 6 years of experience.

Company Description

Founded in 1993, Disability Services of the Southwest (DSSW) is one of the largest providers of support services to people with disabilities and the elderly in the State of Texas. Our services include personal attendant services, nursing and therapies, as well as assistance in home modifications, adaptive aids, job coaching and supported employment.

DSSW is licensed by the state to provide a wide range of services to persons with disabilities including Community Living Assistance and Support Services (CLASS), Consumer Directed Services (CDS), Deaf-Blind with Multiple Disabilities (DBMD), as well as private pay care for the elderly and people with disabilities.

DSSW's programs are designed to assist persons with disabilities in achieving the greatest degree of independence possible within their community. The evaluation process by a nurse and the interview process by a program director and your independent case manager assure that an individual's needs are identified so that they can be met by the services available in each program.

All programs are driven by consumer choice and a people first philosophy.
