Job DescriptionDescription: The Mobile Health Coordinator - Acclaim will support the Acclaim Street Medicine Program and Intensive Primary Care by planning and executing community outreach initiatives for both programs. This position is a frontline public health worker who will act as a patient advocate and liaison to enhance collaboration between the health care system, social services, and the community partners. The Mobile Health Coord - Acclaim also builds individual and community capacity by increasing health knowledge and self-sufficiency, which will lower health disparities in Tarrant County by providing access to services and creating an effective system of chronic disease management for underserved populations.
This role requires a close understanding of the community served as well as JPS Health Network services and programs. If working with Acclaim Street Medicine there will be times when the mobile unit/van for transportation will be utilized. The Mobile Health Coordinator role is expected to drive their own vehicle for transportation.
