Job DescriptionJob Summary: The BH Care Management Coordinator Team Lead provides direct patient care services, including assessment care that promotes positive treatment outcomes and meets the needs of behavioral health patients. This role also involves active participation in performance improvement within the department.
Essential Job Functions & Accountabilities- Performs leadership functions such as assessment of qualifications, skills, and competencies, providing orientation and training, and contributing to performance reviews and hiring decisions.
- Provides clients with self-management skills and educational information to ensure full participation in their treatment and recovery.
- Coordinates care for a behavioral health population across the healthcare continuum.
- Works as part of a multidisciplinary team to develop comprehensive care plans based on systematic assessments of physiological and psychosocial needs.
- Assists the treatment team in recognizing, treating, and managing mental health and psychosocial issues.
- Communicates and collaborates with patients and/or families to ensure understanding of treatment plans.
- Monitors patient progress and adjusts plans of care as needed.
- Documents treatment goals and plans in the electronic medical record, including assessments and relevant clinical documentation.
- Utilizes behavioral health expertise to promote change and improve health conditions.
- Provides care-management skills and knowledge of local resources for outside referrals.
- Advises the care team on clients’ settings, such as other mental health facilities or specialties.
- Assists with authorization processes for treatments.
- Maintains positive, collaborative relationships with other hospital departments.
- Performs other duties as assigned; role duties are subject to change.
- Must be able to perform all role functions with or without reasonable accommodations.
- Work environments may vary; schedule changes and variable hours are possible.
Required Qualifications:
- Master’s Degree in Counseling or Social Work.
- At least one year of experience in a team-oriented, fast-paced, behavioral health environment.
- Over two years of professional social work experience with behavioral health/mental health patients.
- At least one year of team lead/supervisory/management experience.
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
- Current Texas licensure: LMSW, LCSW, or LPC.
- Certification in Non-Aggressive Crisis Management, obtained during orientation and maintained throughout employment.
Preferred Qualifications:
- Over one year of supervisory/managerial experience.
- Licensed Clinical Social Worker (LCSW).
