QMHP – Community Based Youth Services

Texas Care Stop 6 is a Medicaid-enrolled Behavioral Health clinic serving children and families in Fort Worth. Services are delivered in homes, schools, and approved community settings.

This is a community-based field role. Approximately 90% of services are provided in homes and schools. This is not an office-based position.

• Provide Skills Training and Targeted Case Management
• Maintain Medicaid-compliant documentation
• Submit documentation within required time frames
• Coordinate with families, schools, and community partners

• $27 per billable hour
• Contractors who complete 32 or more approved billable hours within a weekly billing cycle are eligible for a $120 weekly performance bonus

• Contractors manage their own schedules based on assigned clients
• Contractors are expected to maintain reliable availability to support client continuity

• QMHP eligible in Texas
• Experience working with youth preferred
• Strong documentation and time management skills
• Reliable transportation
