Synergy Care, Inc. is looking for a full-time physical therapist to join our fun multi-disciplinary team at the luxurious Tradition Senior living IL/AL/MC community in Dallas! We offer outpatient therapy services in-house to Tradition residents as well as to the community.
Key Duties and Responsibilities- Conduct initial patient assessments and evaluations:
Assessing patients' physical capabilities and functional limitations to develop personalized treatment plans. - Develop and implement individualized treatment plans:
Creating tailored exercise programs and therapy plans to address specific patient needs. - Provide hands‑on therapy and therapeutic exercises:
Engaging in hands‑on treatment techniques to promote healing and prevent future injuries. - Educate patients and their families:
Informing patients and their families about treatment plans, home exercise programs, and safety measures. - Monitor patient progress and adjust treatment plans:
Regularly evaluating patients' progress and modifying treatment plans as necessary. - Coordinate with other healthcare professionals:
Collaborating with a multidisciplinary team to ensure comprehensive care and treatment. - Maintain accurate and detailed patient records:
Keeping detailed records of patient interactions and treatment outcomes. - Utilize various physical therapy techniques and equipment:
Employing a range of therapeutic techniques and equipment to support patient recovery. - Ensure patient safety during all therapy sessions:
Taking precautions to ensure the safety of patients during therapy sessions.
Our staff also provide wellness/fitness classes, trainings and in‑services, and personal training for residents in the senior living community and local community.

- Graduate of an accredited school for Physical Therapy.
