Due to our growing patient census, we're currently looking for a Renal Licensed Master Social Worker (LMSW) to join us.

As a Renal Social Worker (LMSW), you will play a key role in supporting our patients' well‑being. Your responsibilities will include:

• Assessing patients' psychosocial status and identifying barriers to successful treatment as part of a comprehensive assessment.
• Collaborating with the multidisciplinary team to develop and implement individualized care plans that improve patient outcomes and quality of life.
• Providing patient and family education on psychosocial issues related to End‑Stage Renal Disease (ESRD) and available support services.
• Working with the interdisciplinary team to educate patients on all treatment options and facilitate transplant referrals.
• Managing the patient admission process to ensure a smooth transition and adherence to the care plan.
• Completing all required documentation to support the billing process.

Education

Master's Degree in Social Work with a concentration in clinical practice from a school of Social Work accredited by the Council on Social Work Education (CSWE).

Current stale licensure or appropriate certification/registration by the slate if required in order to practice social work or be called a Social Worker.

Experience And Required Skills

Experience with renal patients required.

Experience With Helping Patients Deal With Insurance Related Issues.
