Your Impact as a Corporate Clinical Case Manager
As a Corporate Clinical Case Manager supporting our communities, you’ll ensure regulatory compliance, improve care outcomes, and serve as a trusted clinical resource. Your responsibilities will include:

  • Overseeing and supporting case management at assigned facilities
  • Reviewing clinical and therapy documentation for skilled necessity of authorization and continued skilled stays
  • Ensuring timely submission of documentation to insurance portals for continued authorization requests
  • Collaborating with facility teams to address complex care needs and coordination barriers
  • Monitoring skilled residents to assist with appropriate transition of care
  • Providing guidance on payer documentation, coverage verification, and authorization
  • Analyzing trends in care utilization, readmissions, and MCO requirements
  • Preparing analytic reports for facility trends in skilled case mix for your supervisor
  • Acting as a liaison with families, providers, and external partners when escalation is needed
  • Assisting families with appeals when requested

What Makes You a Great Fit

  • RN license, LVN license, or Licensed Therapist (PT/OT/ST/COTA/PTA) required
  • 2–3 years of experience in long-term care, case management, or care coordination
  • Knowledge of Medicaid/Medicare regulations, CMS guidelines, and MCO processes
  • Utilization review or payer documentation experience preferred
  • Strong critical thinking and clinical documentation review skills
  • Ability to work collaboratively with interdisciplinary teams across multiple sites
  • Proficient in EHR systems and Microsoft Office
  • Organized, responsive, and committed to resident advocacy
