Physical Therapist | Full-Time | Outpatient

Fort Worth Physical Therapy, a H2 Health company, is seeking a Physical Therapist to join our Southwest Fort Worth outpatient team. This is an excellent opportunity for a clinician who is passionate about helping patients achieve meaningful functional outcomes while working in a collaborative, patient-centered environment. Whether you're an experienced therapist looking for your next opportunity or a new graduate ready to begin your career, you'll be supported by mentorship, professional development, and the resources of the H2 Health family.

At Fort Worth Physical Therapy, you'll work with a diverse outpatient population and provide individualized, evidence-based care to patients recovering from orthopedic injuries, surgical procedures, sports-related conditions, and other musculoskeletal impairments. Our clinicians focus on building strong patient relationships and developing customized treatment plans that help patients restore mobility, reduce pain, improve function, and return to the activities they enjoy most.

As part of a collaborative team, you'll have access to the support and clinical resources needed to deliver exceptional outcomes while continuing to grow your clinical expertise.

Your Role:

  • Perform comprehensive patient evaluations and develop individualized plans of care.
  • Deliver evidence-based treatment interventions to improve mobility, strength, balance, function, and overall quality of life.
  • Treat a variety of orthopedic, post-operative, sports medicine, and musculoskeletal conditions.
  • Collaborate with patients, families, physicians, and interdisciplinary team members to achieve optimal outcomes.
  • Educate patients on injury prevention, wellness, and home exercise programs.
  • Maintain accurate and timely documentation of patient progress and treatment outcomes.
  • Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time.
  • Utilize the Raintree EMR platform for documentation, scheduling, and patient management.
  • Work within a manageable caseload environment, typically targeting approximately 10-12 patient visits per day for full-time outpatient clinicians.
  • Contribute to a positive clinic culture focused on patient-centered care and clinical excellence.
