Member needs a dependable caregiver to provide hands‑on support and safety supervision due to memory/forgetfulness and limited vision. Caregiver should be comfortable assisting with left‑side weakness support during daily routines, helping the member stay oriented to tasks, and maintaining a calm, structured environment. Ideal caregiver skills include patience, clear communication, strong safety awareness (especially fall prevention), and the ability to provide gentle cueing and step‑by‑step assistance as needed.

Home environment notes:
Member lives with family. Pets are present. Member is a non‑smoker.

Duties will be based on the STAR+PLUS service plan and the member’s day‑to‑day support needs.

• Personal Care Assistance: Support with grooming, hygiene, dressing, and routine self‑care as needed.
• Mobility & Safety Support: Assist with safe movement/transfers as needed; provide fall‑prevention awareness and close supervision due to limited vision.
• Cueing & Supervision: Provide reminders, redirection, and step‑by‑step prompting for daily tasks due to forgetfulness.
• Meal Support: Meal preparation, light cooking, and assistance with safe eating routines as needed.
• Light Housekeeping: Maintain a clean and safe environment (tidying, dishes, wiping surfaces, light laundry, taking out trash).
• Errands/Shopping (if authorized): Grocery shopping and essential errands.
  Driving may be required if included in the service plan.
• Companionship: Provide supportive presence, conversation, and encouragement with daily routines.
• Documentation: Complete required visit notes/timekeeping per agency and program guidelines.
