Salary Range: $60.00 - $72.00 Hourly

Physical Therapist role responsible for delivering quality patient care according to physician orders. Responsibilities include evaluations, treatments, care planning, coordination, health promotion, and family training.

• Provide Physical Therapy services as prescribed by the patient's physician
• Maintain a full-time caseload of at least 30 visits weekly
• Provide a written evaluation defining Physical Therapy needs, goals, and expected outcomes in conjunction with referring physician’s plan of treatment
• Follow policies and procedures and adhere to all applicable patient care standards, licensure regulations, and state laws
• Develop a written plan of care with reasonable goals and outcomes
• Maintain and adjust the plan of care as needed for assigned patients according to identified goals and outcomes
• Maintain patient and company confidentiality
• Maintain effective communication between staff, healthcare team members, and family
• Participate in coordination of services (including discharge planning) with health care team, physicians, patient/guardians, nurses and others

• Master’s or Doctoral degree in Physical Therapy
• Experience with pediatric patients and/or pediatric patients with developmental disabilities
• Great customer service skills
