Overview
Thera Care is seeking a licensed Physical Therapist Assistant (PTA) for a full-time home health opportunity providing in-home physical therapy services to adult and geriatric patients in Keller, TX and surrounding areas (Fort Worth / Keller / Bedford / North Richland Hills / Hurst / Roanoke / Flower Mound). This position offers flexibility, autonomy, and one-on-one patient care in the home setting, with clinicians able to create their own schedules while helping patients improve mobility, strength, safety, and independence in their homes.
Responsibilities- Provide skilled home health physical therapy interventions under the supervision of a licensed Physical Therapist (PT).
- Implement and carry out individualized Plans of Care (POC) developed and delegated by the supervising PT.
- Treat patients with orthopedic, neurological, cardiac, and post-surgical conditions.
- Administer therapeutic exercises, gait training, balance training, transfer training, and functional mobility activities.
- Apply manual therapy techniques and non-complex modalities including heat, cold, electrical stimulation, and other physician-ordered treatments.
- Educate patients and caregivers on home exercise programs (HEP), fall prevention, and safe mobility strategies.
- Monitor patient response to treatment and promptly report changes in condition to the supervising Physical Therapist.
- Complete accurate and timely electronic documentation in compliance with Medicare home health regulations.
- Follow infection control standards and home health safety protocols.
- Participate in QAPI and agency performance improvement initiatives.

- Associate degree from an accredited Physical Therapist Assistant (PTA) program
- Active Physical Therapist Assistant (PTA) license in the state of Texas
- Home health experience (1 year required)
- Home Care Home Base (HCHB) experience (preferred)
- Current CPR certification
- Strong communication, clinical judgment, and time-management skills
- Valid driver’s license, reliable transportation, and current auto insurance
