• During rounds observes, assesses, documents, and reports resident condition/changes.
• Assures implementation of care plans as directed.
• Administers medications as prescribed.
• Conducts rounds with physicians and records visits in clinical records.
• Performs narcotic reconciliation during incoming and outgoing shifts.
• Identifies significant changes in residents’ conditions and takes necessary action per facility policies and procedures.
• Oversees and monitors aides.
• Initiates and completes incident and accident reports per facility policy.
• Discharges residents per facility policy.
• Completes nursing assessment for physician calls prior to contacting a physician about any change in resident condition.
• Communicates with residents’ point of contact when requested for a status update or if there is a change in condition.
• Completes other special projects or functions as assigned by the Director of Nurses and/or Administrator.

• Completed an RN program at an accredited vocational school, college, or university.

• Must be an active Registered Nurse in the state of Texas in good standing.

• Experience working in a skilled nursing facility is preferred.
