Are you a compassionate Registered Nurse with a passion for hospice care? Do you excel in providing holistic, family-centered care to improve the quality of life for your patients? If so, we have an exciting opportunity for you as a Hospice Case Manager in Fort Worth, TX.
In this role, you will be responsible for identifying and addressing the physical, psychosocial, and developmental needs of patients and their families. You will document nursing assessments, develop individualized care plans, and participate in interdisciplinary team meetings to ensure comprehensive care. Your expertise in hospice care will be crucial in managing pain, supporting spiritual and cultural beliefs, and maintaining the dignity of the dying patient.
- Coordinate patient and family services across treatment settings
- Maintain up-to-date electronic medical records in compliance with standards
- Teach appropriate interventions and medication management to patients and caregivers
- Collaborate with the interdisciplinary team to provide holistic care
- Provide care consistent with National Hospice and Palliative Care Organization standards
- Need 2 years experience in setting
- Hospice experience is required
- Experience with HCHB is required
