Licensed Vocational Nurse – Home Health

Primary Responsibilities- Provides high‑quality clinical services within the scope of practice, as defined by state nursing laws, following the plan of care and coordinating with the healthcare team, while adhering to infection control procedures.
- Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record using the agency’s electronic visit verification program (as applicable).
- Participates in the agency’s care coordination process, including timely reporting of patient needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, other clinicians, agency management, case coordinators, and community case managers (as applicable).
- Functions effectively in any home setting, regardless of the client’s age, race, creed, color, sex, disability, or financial condition.
- Current and unrestricted LPN license in the state of practice.
- Current driver’s license and vehicle insurance, access to a dependable vehicle, or reliable public transportation.
- Current CPR certification or ability to complete within 90 days of hire.
- Home‑care experience.
- Ability to manage multiple tasks simultaneously.
- Ability to work independently.
- Good communication, writing, and organizational skills.
- Flexibility to work hours as required to meet client needs.
Annual total cash target pay: $51,738 – $77,606
Per visit point: $24.87 – $37.31
Hourly rate: $29.85 – $44.77
This role is a full‑time position (40 hours weekly). Compensation includes earnings from per‑visit point pay and hourly pay and is based on local labor markets, education, work experience, and performance. The role receives two types of compensation depending on the work performed, with per‑visit pay calculated from productivity points and an hourly rate for certain non‑visit activities.
