Pay: Regular visit $45 to $50 SOC $100 to $120 ROC $90 to $100 Discharge $50 to $60

Responsibilities- Complete OASIS Start of Care, Resumption of Care, Recerts, and Discharges
- Perform head-to-toe assessments and create individualized plans of care
- Teach patients and families on meds, safety, and disease management
- Coordinate with physicians and office clinicians for timely updates
- Document accurately in Well Sky Kinnser and meet compliance standards
Required
- Active Texas RN license in good standing
- Minimum 1 year recent home health RN experience with OASIS
- Proficiency in Well Sky Kinnser
- Reliable transportation and valid drivers license
- Strong communication, organization, and time management
Preferred
- BSN
- Bilingual Spanish or prior wound care experience
