PDN RN/LVN: 6 y/o (GB/Trach/Vent) (7A-7P)

We are seeking a Pediatric Home Health Nurse, RN or LVN, to provide one-on‑one skilled nursing care to medically fragile children in the home setting in City, ZIP. Full‑Time, Part‑Time, and PRN opportunities available.

• Compensation: LVN $22-$28/hr | RN $30-$40/hr

• Provide 1:1 skilled nursing care in accordance with physician orders
• Administer prescribed medications and treatments
• Monitor patient condition and report changes
• Collaborate with care team and family
• Complete accurate clinical documentation electronically
• Assess home health patients to identify physical, psycho‑social, and environmental needs
• Implement, develop, and document the plan of care to ensure quality and continuity of care
• Provide care utilizing infection control measures that protect both staff and patient (OSHA compliant)
• Comply with all VIVA Pediatrics policies and procedures and promote the agency philosophy
• Provide effective communication to patient/family, team members, and other health care professionals via clinical notes, case conferences, and evaluations
• Notify supervising nurse of changes in the client’s condition immediately
• Provide health education to client and/or caregiver
• Document interim changes of plan of care in a timely manner

• Active Texas LVN or RN license
• Current CPR certification
• Valid Driver’s License
• At least 1 year of work experience or 6 months pediatric home health as a licensed RN or LVN
