Pediatric RN Case Manager
The Staff Pad is proud to partner with a respected hospice provider in Fort Worth, TX seeking a compassionate Pediatric RN Case Manager to provide exceptional hospice care to children and their families in the comfort of their homes. Reporting to the Managing Director - Hospice, the Pediatric RN Case Manager, will provide skilled nursing care to pediatric hospice patients while coordinating all aspects of their care.
Working alongside an interdisciplinary team, physicians, and community partners, you'll help ensure every child receives compassionate, dignified, and personalized care.

What You'll Do
- Conduct comprehensive physical, psychosocial, and environmental patient assessments
- Develop, implement, and document individualized plans of care
- Communicate effectively with patients, families, physicians, and the care team
- Provide clinical guidance to Hospice Aides and LVNs and assist with training as needed
- Complete pediatric admissions and prioritize patient scheduling
- Participate in assigned on-call rotations for emergency patient support
- Maintain accurate documentation and ensure compliance with infection control and agency policies
- Support ongoing quality improvement and professional development
- Perform other duties as assigned
What We Are Looking For
- Graduate of an accredited Diploma, ADN, or BSN Nursing program
- Current Texas Registered Nurse (RN) license
- Minimum one (1) year of pediatric RN experience in a clinical setting required
- Hospice, Critical Care, or Emergency Room experience preferred
- Current CPR certification
- Valid driver's license, reliable transportation, and current auto insurance
- Current TB testing and Hepatitis consent/declination documentation
- Ability to pass a criminal background check

- Field-based role with local travel and PRN on-call rotation
- Ability to stand, walk, lift, and safely transfer up to 50 lbs
- Perform clinical procedures requiring manual dexterity and hand-eye coordination
