Frontpoint Health is seeking an LVN for home health visits in the Dallas–Fort Worth area. The role is PRN/per visit, performing skilled nursing tasks in patients' homes and coordinating care with the interdisciplinary team.
Prior home health experience is required, with active licensure and CPR/TB/insurance documentation. The LVN will observe patient conditions, administer medications, document care, and participate in ongoing staff training and QAPI initiatives.
